As a result, the risk of fractures increases. It is a public health problem as osteoporotic fractures have a significant impact on an individual’s quality of life, premature morbidity and mortality. Resistance training is a method that can improve most of the modifiable risk factors for fracture and allow the development and maintenance of optimal bone strength throughout life. The purpose of this systematic review was to review research findings on the effects of exercise interventions that progressively incorporate resistance training in individuals with primary osteoporosis. PubMed and CINAHL databases were searched for the period from January 2003 to August 2024. Five randomised controlled trials with PEDro scores between 4 and 8 were included in the review. The studies included older adults with primary osteoporosis. Resistance exercise improved muscle strength (three out of four studies) and balance (four out of five studies) in a statistically significant way. Resistance exercise is effective for preventing falls and consequent fractures in older adults with primary osteoporosis. To be effective, it must be accompanied by other forms of exercise interventions. It is particularly important that exercise is individualized, safe, supervised, progressive and sustained over the long term</dc:description><dc:description xml:lang="sl">Osteoporoza je sistemska skeletna bolezen, ki povzroča zmanjšano mineralno kostno gostoto in propadanje kostne strukture, zaradi česar se zmanjša jakost kosti. Posledično se poveča tveganje za zlome. Gre za javnozdravstveni problem, saj osteoporotični zlomi pomembno vplivajo na kakovost življenja posameznika, prezgodnjo obolevnost in umrljivost. Vadba proti uporu je metoda, ki lahko izboljša večino spremenljivih dejavnikov tveganja za zlome ter omogoča razvoj in ohranjanje optimalne jakosti kosti skozi celotno življenje. Namen sistematičnega pregleda je bil pregledati izsledke raziskav o učinkih vadbenih intervencij, ki progresivno vključujejo vadbo proti uporu pri posameznikih s primarno osteoporozo. Pregledane so bile podatkovne baze PubMed in CINAHL za obdobje od januarja 2003 do avgusta 2024. V pregled je bilo vključenih pet randomiziranih kontroliranih raziskav z ocenami med 4 in 8 po lestvici PEDro. Objavljene so bile med letoma 2003 in 2022. Raziskave so vključevale starejše odrasle s primarno osteoporozo. Vadba proti uporu je statistično značilno izboljšala mišično jakost (tri izmed štirih raziskav) in ravnotežje (štiri izmed petih raziskav). Progresivna vadba proti uporu je učinkovita za preprečevanje padcev in posledičnih zlomov pri starejših odraslih s primarno osteoporozo. Da bi bila učinkovita, jo morajo spremljati druge oblike vadbenih intervencij.
