This can be minimized by iron chelating therapy. The assessment of iron overload isessential to assign and adjust this treatment. Serum ferritin is a widely used marker for the estimation of iron stores. However, a variety of conditions alter the relationship between serum ferritin levels and body iron stores. Soluble transferrin receptors in serum (sTfR) reflect the degree of erythropoiesis and the status of body iron. The data on usage of serum transferrin receptors for the assessment of iron overload are sparse. The purpose of our work was to estimate iron overload in transfusion dependentpatients and to assess the combinative value of serum transferrin receptors and ferritin in prediction of iron load. Methods Serum levels of iron, transferrin saturation, ferritin ctnd sTfR were evaluated for 36 transfusion dependent patients. Twenty of them had myelodysplastic syndrome (MDS). Body iron (mg/kg) was calculated from the sTfR/ferritin ratio with the use of regression formula developed by Cook et al. Levels of serum parameters and calculated body iron were compared with estimated iron overload. We used absolute number of transfused units of blood as a measure of iron overload. (Abstract truncated at 2000 characters)</dc:description><dc:description xml:lang="sl">Izhodišča Neželena posledica kroničnega zdravljenja s transfuzijami eritrocitov je čezmerno kopičenje železa v telesu, ki lahko povzroči okvaro srca, jeter in endokrinih žlez. Temu se lahko izognemo z zdravljenjem s helatorji. Za pravilno odločitev o začetku zdravljenja in za oceno uspešnosti so potrebne meritve obremenitve z železom. Najbolj običajen način merjenja je določitev koncentracije feritina v serumu. Ker vrsta stanj neodvisno od količine železa v telesu vpliva na koncentracijo feritina, preiskava pogosto ni natančna. Koncentracija topnih transferinskih receptorjev (sTfR) v serumu zrcali obseg eritropoeze in količino železa v telesu. V literaturi so podatki o uporabi sTfR za oceno obremenitve z železom redki. Namen našega dela je bil oceniti obseg čezmernega kopičenja železa v telesu pri bolnikih na kroničnem zdravljenju s transfuzijami eritrocitov in ugotoviti, ali dodatno merjenje koncentracije sTfR izboljša oceno obremenitve z železom. Metode Pri 36 bolnikih smo določili koncentracije železa zasičenje transferina z železom, feritina in sTfR v serumu. Dvajset bolnikov se je zdravilo zaradi mielodisplastičnega sindroma (MDS). Poleg omenjenih meritev smo iz kvocienta sTfR/feritin izračunali količino železa v telesu v mg/kg. Za to izračun smo uporabili regresijsko enačbo, ki jo je razvil Cook s sodelavci. Omenjene koncentracije v serumu in izračunano količino železa v telesu smo primerjali zocenjeno obremenitvijo z železom. Za oceno obremenitve z železom smo uporabili število enot eritrocitov, ki jih je bolnik prejel med zdravljenjem.
