In the last decades a palliative care has been well established inthe majority of West European countries. However, majority of these countries are not able to follow needs for palliative care because of demographic changes (older population), changes of morbidity pattern (increaseof chronic progressive diseases) and social changes (disability of families to care for their relatives at their homes). Research is showing evidence on palliative care effectiveness at end of life and in bereavement. There is still a great need for healthcare professionals' change in their attitudes, knowledge and skills. In many National strategic plans (United Kingdom, Ireland, Sweden, Australia, New Zealand and Canada) palliative care becomes a priority in the national public health. New organizationalplanning supports establishement of palliative care departments in hospitals and other healthcare settings and consultant teams at all levels of healthcare system. Hospices, caritative and independent organizations, will remain as a source ofgood clinicalpractice and philosophy of care at the end of life also in the future</dc:description><dc:description xml:lang="sl">Izhodišča. V zadnjih desetletjih se je organizirana paliativna oskrba znotraj zdravstvenega sistema in tudi zunaj njega v deželah Zahodne Evrope močno razširila. Kljub temu v večini držav ne uspejo slediti potrebam zaradi demografskih sprememb staranja prebivalstva in spreminjanja vzorca bolezni, konaraščajo predvsem kronične napredujoče bolezni ter socialne spremembe, ko družine mnogokrat ne morejo oskrbovati svojcev na svojem domu. Raziskave dokazujejo, da se z uvedbo organizirane paliativne oskrbe kakovost ob koncu življenja bolnikov in žalovanje svojcev izboljša vendar je še potrebno tudi v deželah z dolgo tradicijo spodbuditi spreminjanje stališč znanja in veščin vseh zdravstevnih delavcev. Zato v mnogih nacionalnih strateških načrtih (Anglija, Irska, švedska, Avstralija, Nova Zelandija in Kanada) paliativno oskrbo postavljajo kot prednostno nalogo pri načrtovanju nacionalnega javnega zdravstva. Nova organiziranost omogoča in spodbuja ustanavljanje paliativnih oddelkov v bolnišnicah in domovih za starejše občane ter konzultacijskih paliativnih timov na vseh ravneh zdravstva.
