Gastric cancer is the sixth most common cancer in Slovenia. The known risk factors for gastric cancer are: Helicobacter pylori infection (H. pylori), E-catherin mutation, proinflammatory cytokine polymorphisms, certain foods (salt, nitrosamine, antioxidants), pernicious anemia, 20 years post gastric partial resection, gastric adenoma and hamartoma, male sex and smoking. From 65 % to 80 % of noncardia gastric cancer are late consequences of H. pylori infection. Half of the World population is infected with H. pylori, but only 1 % will develop gastric cancer. Last year, Asian Pacific Cancer Concensus Conference recommended population - based screening and treatment of H. pylori infection in countries with gastric cancer incidence more than 20/100 000. Untill this approach becomes a part of daily clinical practice, endoscopic and histologic surveillance should be done in all patients with premalignant gastric lesions (atrophy, intestinal metaplasia and dysplasia). Histologic caracteristics and recommendations for endoscopic and histologic surveillance are presented"},{"@xml:lang":"sl","#text":"Izhodišča: Rak želodca je drugi razlog smrti zaradi raka na svetu. V Sloveniji je rak želodca po incidenci in po vzroku smrti na šestem mestu. Med nevarnostne dejavnike za nastanek raka želodca spadajo: okužba z bakterijo Helicobacter pylori (H. pylori), mutacije gena E-catherin, polimorfizem provnetnih citokinov, hrana (sol, nitrozamini, vnos antioksidantov), perniciozna anemija, stanja 20 let po resekcijah želodca, bolniki z adenomi in redko hamartomi želodca, moški spol in kajenje. Ocenjujemo, da je 65 % do 80% vseh primerov raka želodca brez raka kardije posledica okužbe z bakterijo H. pylori. Polovica prebivalcev sveta je okužena z bakterijo, rak želodca pa se pojavi le pri 1 % okuženih. Zaključki: Osnovni pristop k zdravljenju raka želodca v državah, kjer je incidenca raka večja od 20/100.000 letno, bi moral biti presejanje prebivalstva na okužbo z bakterijo H. pylori in zdravljenje okužbe. To je tudi sklep Azijsko-pacifiškega konsenza iz leta 2008. Dokler ta pristop ne bo uveljavljen v praksi, pa je potrebno posebno pozornost posvetiti endoskopskemu in histološkemu spremljanju bolnikov s predrakavimi spremembami želodčne sluznice: atrofijo, intestinalno metaplazijo in displazijo.
