Chronic obstructive pulmonary disease (COPD) shares many clinical and pathophysiologic features with asthma. Yet, COPD and asthma are different diseases, with their own pathogenesis, clinical picture, management and prognosis. Still there are not so few diagnostic mistakes between asthma and COPD. Methods. The possibilities of differentiating between asthma and COPD were stressed. Differences in symptoms or in therapy between asthma and COPD were shown. Conclusions. Asthma and even more COPD are too often recognized only in late, severe forms. Important clinical benefits of early diagnosis of asthma or COPD were shown, It was recommended, that spirometry should become routine diagnostic test on the level of primary health care"},{"@xml:lang":"sl","#text":"Izhodišča. Kronična obstruktivna pljučna bolezen (KOPB) deli z astmo mnoge lastnosti. Bolezni označujejo naduha, kašelj in izkašljevanje, patofiziološko pa zapora dihal. Vendar sta astma in KOPB povsem različni bolezni, z drugačno patogenezo, klinično sliko, zdravljenjem, preventivnimi ukrepi in prognozo. Zato je razlikovanje med boleznima nadvse pomembno. Metode. V prispevku so opisane razlike v etiopatogenezi med astmo in KOPB. Podrobneje je opisan postopek diagnoze KOPB s poudarkom na možnosti zgodnje diagnoze KOPB. Poudarjene so razlike med simptomi astme in KOPB ter podobnosti in razlike primedikamentnem zdravljenju astme ali KOPB. Posebej so opisane posebne metodezdravljenja KOPB, kot so opustitev kajenja, respiratorna rehabilitacija ter trajno zdravljenje s kisikom na domu. Poudarjene so naloge zdravnikov družinske medicine pri obravnavi bolnikov z astmo in bolnikov s KOPB. Zaključki. Astmo in še pogosteje KOPB odkrijemo kasno. Ocenjujejo, da je diagnosticiranih le približno polovica bolnikov s KOPB. V prispevku so opozorili na možnosti razlikovanja med astmo in KOPB. Poudarili so viden pomenzgodnje diagnoze KOPB.
