The clinical course is very diverse, from a limited non-progressive involvement of the pulmonary interstitium to a life-threatening fulminant disease. While immunomodulatory drugs are usually the first-line treatment, antifibrotics have recently proved to be effective in progressive pulmonary fibrosis. It is often challenging to decide whom, when, and how to treat intensively. The following paper presents the conclusions of a multidisciplinary team of Slovenian rheumatologists and pulmonologists. The suggested diagnostic approach and treatment represent good clinical practice based on the available studies and clinical data, focusing primarily on patients with systemic connective tissue diseases and rheumatoid arthritis"},{"@xml:lang":"sl","#text":"Vnetne revmatične bolezni so heterogena skupina bolezni, ki lahko prizadenejo tudi pljučni intersticij. Tako kot je pogo-stnost intersticijske pljučne bolezni (IPB) v okviru vnetnih revmatičnih bolezni variabilna, je tudi klinična slika zelo razno-lika: od omejene nenapredujoče prizadetosti pljučnega intersticija do fulminantno potekajoče življenje ogrožajoče bo-lezni. Podatki za diagnosticiranje in zdravljenje IPB v sklopu vnetnih revmatičnih bolezni so razmeroma skopi. Temeljno zdravljenje praviloma predstavljajo imunomodulacijska zdravila. Nedavno so se jim pri progresivni pljučni fibrozi pridružili še protifibrotiki. Odločitev pri kom, kdaj in kako intenzivno zdraviti IPB pogosto predstavlja izziv. V prispevku podajamo sklepe multidisciplinarnega posvetovalnega sestanka slovenskih revmatologov in pulmologov, ki po mnenju avtorjev gle-de na dostopne podatke pomenijo dobro prakso pri obravnavi tovrstnih bolnikov.
