Patients and methods From July 2004 until the end of 2006, 12 patients with 15 intraarticular fractures of the mandibular condyle head had been treated at the Department of Oral and Maxillofacial Surgery in Ljubljana, using the preauricular approach. Results Eleven of the 12 patients can open their mouths for 40 mm or more, none of them experience significant deflections of the chin. We didn't have a single case of postoperative weakness of the temporal branch o f the facial nerve. None of the patients experience pain upon rest, palpation or chewing. Conclusions Using the appropriate surgical approach and technique makes the surgical procedure safe and leads to good resultsIzhodišča Intraartikularni (znotrajsklepni) zlomi glavice sklepnega (kondilarnega) odrastka mandibule v temporomandibularnem sklepu (TMS) so na večini ustanov, kjer se ukvarjajo z zdravljenjem zlomov obraznega skeleta, kontraindikacija za kirurško zdravljenje. Bolniki in metode Od julija 2004 do konca leta 2006 smo na KO za maksilofacialno in oralno kirurgijo v Ljubljani spreavrikularnim pristopom kirurško oskrbeli 12 bolnikov s 15 intraartikularnimi zlomi glavice sklepnegu odrastka mandibule. Rezultati Enajst od 12 bolnikov odpira usta za 40mm ali več, nihče od njih pri odpiranjunima bistvene lateralne deviacije brade. V nobenem primeru ni prišlo do začasne pareze temporalne veje, facialisa. Nihče od bolnikov ne navaja bolečin v mirovanju, ob palpaciji ali pri žvečenju hrane.
